$375.00¹ Please send check, playable to New Berlin Heat, along with a completed registration form to: Tammy Lewis, Tournament Director 18565 W. Evergreen Place New Berlin, WI 53146 |

| Age Groups: | Boys: 2nd Grade (U8), 3rd Grade (U9), 4th Grade (U10), 5th Grade (U11), 6th Grade (U12), 7th Grade (U13), & 8th Grade (U14) Age cut-off for above categories: May 1st |

All teams must submit their rosters by July 1! |
| Location: | New Berlin, Wisconsin (Actual Field Locations Provided Closer to Tournament Date.) |
| Format: |
Pool Play - 3 game guarantee (Weather Permitting!) 4th game possible for top 4 teams in each Age Group |
| Awards: | 1st & 2nd Place for each Age Group and 1st Place Consolation. |
| Concessions: | Provided by the host. Please no carry-ins of food or beverage! |
